Prior to admission, a professional clinical assessment by the Licensed Social Worker and Director of Nursing will be completed in your home or at Pleasant View to ensure successful daily living.

Care plans could include medication reminders, transportation, showering or bathing, dressing, escorts for activities and meals within Pleasant View, eating assistance, toileting and incontinence care, wake up and turn in assistance, overnight checks, injections and wound care.
